Mental Health Act Schedules

With the entry into force of the Mental Health Act 2012 the following schedules (forms) need to be filled in by medical practitioners according to care requirements.

Schedules to be used by the Primary Care Physicians/General Practioners/Family Doctors are:

 
Schedules to be used by Specialists in Psychiatry are:


For the involuntary admission of minors, Schedule 13 / Skeda 13 by a specialist with clinical experience of working with minors with mental health problems is to accompany Schedule 2 / Skeda 2.

These Schedules may be accessed, filled, signed and saved electronically or else they may be accessed, printed and filled in and signed manually as per instructions below.   

 
Instructions to fill in Schedules electronically:
 
  1. Click on the desired schedule and open
  2. Click on Sign (top right corner)
  3. Click on Add text and type in the required information
  4. Click on Add checkmark to put in relevant checkmarks where necessary
  5. Click on Place Signature to include an electronic image of your signature
  6. Click on Signed Proceed to send
  7. Click on Save a Copy
  8. Name File (suggestion [Surname] [Name] [ID no.] of patient) and save in your personal records
  9. If SENDING ELECTRONICALLY: Open a new email, attach filled and signed schedule and send to designated recipient depending on case
                                                         OR
10. If SENDING HARD COPY​: Print a hard copy and send to designated recipient depending on case 
 

Instructions to fill in Schedules manually:
 
  1. Click on the desired schedule and open
  2. Print the schedule, fill in all details and sign
  3. Send manually to the designated recipient depending on case
                                                           OR
4.   Scan the completed form and send as an attachment to the designated recipient depending on case.

  • Schedule 1 - Application to restrict freedom of communication
  • Schedule 2 - Application for a person to be admitted involuntarily for observation in a licensed mental health facility

  • Schedule 10 - Application for revocation of a community treatment order
  • Schedule 11 - Certification of lack of mental capacity
  • Schedule 12 - Application for revocation of a certificate of lack of mental capacity
  • Schedule 13 - Notification by specialist with clinical experience of working with minors with mental health problems for an involuntary admission for observation of a minor
  • Schedule 14 Part 2 - Application for invasive or irreversible treatment
  • Schedule 15 - Application for leave under article 37 for a person detained under court order
